As has been mentioned before, AAI Speed Shop off of 60th and Marshall will also custom make hoses:

In addition to that, you can buy the different sized Hydraulic line/hose and simply purchase Earl's Fittings and make your own. Simple way to do it. I used this system for the Hydro assist hoses on my rig and it has worked fine for years. Carrying a couple of additional Earl's Fittings and a couple of lengths of hose in the trail kit also assure that trail repairs can be easily done.
