What it would take to convert a van like this to 4wd?

There are companies that convert the E series vans. Typically they convert them brand new. They are often times the diesel version too. The cool part about buying one from these conversion companies is they also typically have a pop-top, so you get more room. They would also be 1-ton drivetrain for added durability. We actually camped near one in the middle of nowhere in Canyonlands. I think they said new it was over $100k, but they had just bought it used and it was only a couple years old.

In regards to converting, you are likely going to replace the rear axle and lift it to match the front axle. Cut the entire front suspension off and convert to leaf springs. Replace the transmission with a 4wd transmission and t-case. New driveshafts needed. Really you are going to gut the suspension and drivetrain. There are some companies that sell conversion kits:

Personally I would try to find one already converted to 4wd, and then do the camper conversion to your liking. It is probably cheaper, and you can make it exactly ho you want it. I think Quigley is the best known conversion company. Here are some for sale by owner already converted to 4wd, one of the cheapest being in Golden:
