First off, thank you to all that came and helped out on the 2016 fall cleanup run. If not for you we'd not be able to adopt these two beautiful trails. I'd like to apologize again for not having sign up sheets on site Sunday as they were misplaced during our recent move.

Secondly, If you could please take a moment and click on the attachment posted here, print one copy and fill in just one section of the form (the first 3 lines) and then scan or take a picture and email it to me directly at azshopadmin@hotmail.com to protect your privacy, it would be much appreciated!

Lastly, it is extremely important that we get these forms back to ensure that we as individuals as well as Front Range as a group get credit with the Forest Service.

If you have any questions of trouble please let me know. Thank you in advance and once again, my apologies!

Chad

I'll work with Chad to get a PDF version that would be easier to download.

I believe this is the correct form:

http://www.nrcs.usda.gov/Internet/FS...1p2_021877.pdf

AND it's a "fillable PDF" form for the win!

Does someone turn this in? I don't recall seeing Front Range listed in the last few years.

We already discussed the reporting. I only reported to the USFS not to any other organization. ;)

USFS does not send their reports to the Adopt-a-Trail folks. IF people want FR4x4 volunteer hours to be visible to the general public they have to be submitted via the link.

Personally, I don't see a need for the listing of us on that site but maybe others will diagree.

Just tossing it out there!

Just to make sure - it says expires 12/2013....that gonna cause any problems?

Also, anything specific to put in the first line for Site/Trip leader and Agency blanks. I intend to fill it out, print and sign, then scan and email back. Is there a better way to do it?

I believe this is the correct form:

http://www.nrcs.usda.gov/Internet/FSE_DOCUMENTS/nrcs141p2_021877.pdfNo, that is the form the club's representative signs, making them the contact person for the USFS Volunteer Coordinator, and committing the group or individual to the terms of the Volunteer agreement.

"FS-1800-7b (revised 3/04)" is the Word document I have been filling out for volunteer days. It looks very much like the "OMB D596-0080" form that Chad posted.

Sounds like no one exactly sure of what exactly to do here, if some one has done this before, please step up and provide all necessary info so we can turn this in.
Thanks

The form that Chad posted is the right form. I have a stack of them at home. I can take
a photo of one that I filled out and send it to you Chad. Or just mail it to your address.

Chad sent the form to me but it came through as a zero byte file - Word DOC format. I asked him to re-send. I'll put up a PDF when I receive it.

I dug a copy from my web mail, have on my work computer. I need an email address that I can send an attachment to.

Thanks Mike.

Here's a PDF of the form Chad has in the first post of the thread:

Download / print / fill-out / Scan (or take a picture of it) / eMail back to Chad

I've been getting the emails, thank you so much everyone, I apologize again, 2 moves since the spring clean up really messed me up and having the nearest copy machine I know of clear over in Silverthorne (other than the Idaho springs hardware store which is closed on Sunday) makes things tough as well as my printer being in storage. I appreciate everyone's hard work on the clean up and ESPECIALLY appreciate the EXTRA work of signing in. I'll figure out a way to thank everyone at next springs' clean up. I'm about to go start pulling the emails and putting the sign in's together on just a few sheets to get them emailed into the forest service.

Thank you thank you ONE MORE TIME, this group is the best!!

Chad

It'll all work out - that's the group process.

I still have pix to forward to you - one of the bullet riddled bird house, one of the new stay-the-trail sign put up, one of a trail marker not set into soil (propped up with rocks).

Oh, and that sign-up form. It's been sitting on the scanner the past two days...

I will have mine sent in as soon as I get home.
